Job Description:
A company is looking for a Telephonic Nurse Care Associate to
provide telephonic interventions for patients with chronic
conditions.
Key Responsibilities
Complete patient interventions and follow up on biometrics
readings
Engage patients in their care using Motivational Interviewing
techniques
Document patient information accurately and maintain clinical
charts
Required Qualifications, Training, and Education
Nursing degree from an accredited college or university or
equivalent experience
Minimum of 5 years of broad clinical experience
At least 3 years of experience in home care, care coordination, or
telehealth
LPN/LVN licensure in appropriate state(s), active and in good
standing
Strong computer skills and ability to navigate multiple
programs
